Diabetes Management Incentive Program 

Earn rewards and improve your health when you take steps to manage your diabetes.

Get Started with the BHA

Our Diabetes Management Incentive Program is designed to help educate and support you as you take an active role in managing your diabetes.

Up to two members on a contract, age 18 and over, can participate in the program and may earn up to $100 on their MyBlue® Wellness Card.* You can use the card to pay for qualified medical expenses, such as copayments and prescriptions.

Here’s How to Participate

  1. Take your Blue Health Assessment
    Complete the Blue Health Assessment and indicate that you have Type 1 or Type 2 diabetes.

  2. Submit your A1c test results
    Submit your A1c test results between January 1 and June 30, 2017 to earn $25.

  3. Lower your A1c or attend nutritional counseling
    Submit a second A1c test result between July 1 and December 31, 2017. If your A1c is lower than 8%, you’ll get an additional $75 on your MyBlue Wellness Card.

    If your second A1c test result is 8% or above, you can still earn $75 when we receive claims showing that you attended 3 nutritional counseling sessions during the calendar year, which are covered in full for both Standard and Basic Option members when you visit a Preferred provider.

    Members can submit their results via upload, mail or fax or ask their provider to mail or fax their results.
