You’re required to use Preferred providers to receive benefits. With no deductible, pay $30 for each office visit to a Preferred primary care provider and $40 for each visit to a Preferred specialist.

FEP Blue Focus

Your Needs. Your Budget.

Get quality healthcare coverage in-network plus budget-friendly benefits. You can enroll during 2018 Open Season for coverage that starts January 2019.

Medical Benefits

You must stay in-network, but no worries—96% of hospitals, 95% of doctors and over 65,000 Preferred retail pharmacies are in our network. Plus, there’s no referral needed to see a specialist, and your coverage travels with you overseas.

What's Not Covered

You get plenty of benefits with FEP Blue Focus. However, there are some things the plan doesn't cover. These include dental care, Non-preferred drugs, skilled nursing facility care, hearing aids and long-term care.

Prescription Drug Coverage

FEP Blue Focus covers certain drugs—mainly generics and some Preferred brand name and specialty drugs. It doesn’t cover all FDA-approved drugs. There are two covered drug tiers and the amount you pay for your drug depends on the tier it’s in.

Core Benefits

Your Core benefits provide coverage for all the essentials of good preventive health. We cover all the benefits in this category at little or no cost to you when you visit Preferred providers. If 18 or older, the contract holder and spouse can even earn rewards, like a discounted meal kit delivery service subscription or free health club membership, just for getting an annual checkup. All you have to do is visit your primary care doctor.

Non-core Benefits

Non-core benefits include all primary care and specialists visits beyond the first ten combined, lab and diagnostic services, allergy treatments, surgeries and emergency care. For all Non-core benefits, you pay 30% of our allowance after you meet your calendar year deductible.

Wrap Benefits

Wrap benefits give you access to additional benefits not covered in Core or Non-core. These include things such as maternity care and physical therapy. These services are not subject to the deductible.
