Having high blood pressure (also known as hypertension) significantly increases your risk for serious health conditions, such as heart attack, stroke, heart failure and kidney disease. The best way to know if you have high blood pressure is to visit your doctor and have your numbers checked on a regular basis.
The Hypertension Management Program can help Standard and Basic Option members monitor their blood pressure numbers at home with a blood pressure monitor at no out-of-pocket cost.* Eligible FEP Blue Focus members can get one as well.
Here's how it works:
- Complete the Blue Health Assessment and answer “yes” to the question “Has your doctor ever diagnosed you with high blood pressure?”
- After we receive a claim from your provider noting you have hypertension, you will automatically be enrolled in the Hypertension Management Program.
- We’ll send you a letter with instructions on how to get your blood pressure monitor.
Having a blood pressure monitor at home gives you the convenience to check your blood pressure as often as your doctor recommends and easily track your numbers, which you can share with your doctor. As part of this program, you’re eligible to receive a new blood pressure monitor every two years.
