Basic Option Members with Medicare Part A and B can receive up to a $600 Medicare reimbursement account.

Register for Your Account

As an active Basic Option member enrolled in Medicare Part A and B, you are eligible to be reimbursed up to $600 per calendar year for your Medicare Part B premium payments. Each eligible member on a contract has their own $600 benefit.

Register for your Medicare reimbursement account online. If you have any questions, call 1-888-706-2583 weekdays from 8 a.m. and 8 p.m. Eastern time, excluding holidays.

  • You will need your 4-digit ID code. Your ID code is a combination of your day of birth (DD) and the last 2 digits of your SSN. For example, if you were born on the 8th day of the month and the last 2 digits of your SSN are 12, your ID code is 0812.
  • After you register, you will get 24/7 access to your account and you will be able to receive important updates and alerts.

Provide proof of payment

No matter how you decided to submit your claim, you must provide proof of your Medicare Part B premium payment. Examples include:

  • Social Security Cost of Living Adjustment (COLA) statement or Annuity Statement
  • A copy of a credit card statement*
  • A copy of a bank statement*

Submit a claim

There are 4 ways to submit your claim. You have until December 31 of the following benefit year to submit your claim for reimbursement.

  1. Fax to 877-353-9236
  2. Mail to P.O. Box 14053, Lexington, KY 40512
    1. To submit a paper claim by fax or mail, download a claim form and follow the instructions on the form.
  3. Upload through WageWorks EZ® Receipts mobile app, available at the App Store® and Google Play
  4. Upload through the WageWorks website

Information required by the IRS

Make sure your submitted documents include the five following pieces of information as required by the IRS:

  • Name
  • Date of payment
  • Detailed description
  • Provider name (which is Medicare in this case)
  • Proof of payment

Reimbursement

Most claims will be reviewed within one to two business days after they have been received. Upon approval, you will receive reimbursement by direct deposit or check, depending on how you set up your account.
