Your health matters. Take the 2018 Blue Health Assessment (BHA) to address health risks before they become issues and earn $50 on your MyBlue® Wellness Card.* Answer simple questions, and in just 10 minutes you’ll receive a clear, concise, personalized approach to a healthier you. You can even take the BHA multiple times throughout the year to update your answers, add information and see your progress.
The BHA is the first step in our Wellness Incentive Program. Once you’ve completed it, you may also be eligible to earn an additional $120 for completing up to three Online Health Coach goals related to nutrition, weight management, exercise, stress management, emotional health—and managing certain chronic illnesses.
Plus, if you indicate you have specific conditions on your BHA, you may be eligible to earn even more rewards through our other programs.
Pregnancy Care Incentive Program: Offers expectant mothers support and rewards throughout their pregnancy, including a free Pregnancy Care Box and $75 after their first prenatal care visit in the first three months of pregnancy.
Diabetes Management Incentive Program: When you complete the BHA and indicate that you have Type 1 or Type 2 Diabetes, you may be eligible to earn up to $100 for taking steps to manage your condition, such as having your A1c tested and getting nutritional counseling.
Hypertension Management Program: When you indicate that you have high blood pressure on the BHA and we receive a hypertension-related claim for you, you may be eligible to receive a free blood pressure monitor.
Tobacco Cessation Incentive Program: After you take the BHA and indicate that you use tobacco, set a goal to quit to be eligible to receive prescription and/or over-the-counter tobacco cessation products at no cost.
Any rewards earned will be loaded onto your MyBlue® Wellness Card. You can use these funds for qualified medical expenses such as co-payments and prescriptions.
