Blue Health Assessment
Get a snapshot of your overall health based on information you provide in a guided survey.
Taking charge of your health is important. That’s why Standard and Basic Option members can earn rewards for working hard as part of our Wellness Incentive Program.
There are two ways you can earn rewards*:
- Earn $50 when you complete your Blue Health Assessment (BHA)
- Earn up to an additional $120 when you complete certain goals with our Online Health Coach
Once you’ve completed the BHA, you may also be eligible to earn an additional $120 on your MyBlue® Wellness Card for completing up to three Online Health Coach goals related to a healthy lifestyle in the areas of Nutrition, Weight Management, Exercise, Stress and Emotional Health, as well as goals related to managing Coronary Artery Disease, Congestive Heart Failure, Chronic Obstructive Pulmonary Disease (COPD), Asthma and Hypertension.
You’ll receive $40 for each eligible goal you complete, up to three, for $120 total. All three goals must be completed within the calendar year.
Any rewards earned will be loaded onto to your MyBlue Wellness Card. You can use these funds for qualified medical expenses such as copayments and prescriptions.
* You must be the contract holder or spouse, 18 or older, on a Standard or Basic Option Plan to earn incentive rewards.
