2008 Vision Care And Eyewear Affinity Discount Program.

Few things in life are as precious as good vision. As a Blue Cross and Blue Shield Service Benefit Plan member, you are entitled to receive routine eye exams, frames, lenses, conventional contact lenses and laser vision correction at a savings when you use participating Davis Vision providers for your eye care needs. If you have FEP BlueVision, we will coordinate benefits to maximize your savings.

Check Out The Savings When You Use A Participating Davis Vision Provider:

• For frames costing $70 or less you pay $40.

• For frames costing more than $70 or less you pay $40 plus 90% of the amount over $70.

• For Single Vision lenses (uncoated plastic) you pay $35.

• For Bifocal lenses (uncoated plastic) you pay $55.

• For Trifocal lenses (uncoated plastic) you pay $65.

• For Lenticular lenses (uncoated plastic) you pay $110.

The cost for the following lens options are added to the lens prices listed above.

• Standard Progressive $60/$75.

• Premium Progressive $110/$125.

• Glass lenses $18.

• Polycarbonate lenses $30.

• Scratch-ressistant coating $15.

• Glare-resistant treatment $45.

• Ultraviolet (UV) coating $15.

• Solid tint $10.

• Gradient tint $12.

• Photogrey Extra® $35.

• Transition® Lenses $65.

Special lens designs, material, powers and frames may require additional out-of-pocket costs.

For eye examination you receive 15% off the Usual & Customary charge or $5 off the retail cost.

For refraction only, you pay $20 when the examination is also covered by Medicare.

For contact lens examination you receive 15% off the Usual & Customary charge or $10 off the retail cost.

For conventional contact lenses you receive 20% off the Usual & Customary charge.

For disposable/planned replacement contact lenses you receive 10% off the Usual & Customary charge.

For non-prescription sunglasses you receive 20% off the Usual & Customary charge.

For laser vision correction you receive up to 25% off the provider's Usual & Customary change or 5% off any special offer.

Show Your ID Card And Save.

The Davis Vision affinity program is not an insurance program, so you will not have to file claims. All charges are handled directly between you and the Davis Vision participating provider. All you do to receive the discounts is use a participating provider and present your Blue Cross and Blue Shield Service Benefit Plan identification card.

It is easy to find a Davis Vision provider that's close to you - even when you are away from home. Members have access to over 26,000 providers nationwide. These provider locator services are available 24 hours a day, seven days a week. (Please note: When you choose this link, you are leaving fepblue.org and going to a new web site. Please Note: Benefits under this vision program are neither offered as part of nor guaranteed under the Blue Cross and Blue Shield contract with the US Office of Personnel Management also known as OPM. It is available to all enrollees and family members covered under the Service Benefit Plan. The cost of these benefits is not included in your premium. These benefits are offered to our members to provide preventive and routine care, and as an added value to their Service Benefit Plan coverage.

This vision plan covers only routine vision needs. Benefits for the medical and surgical treatment of diseases and conditions of the eye are available as described in the 2008 Blue Cross and Blue Shield Service Benefit Plan brochure.
