Federal DentalBlue is an optional dental benefit program with an additional premium that supplements the dental benefits included in your Blue Cross and Blue Shield Service Benefit Plan coverage.

How To Enroll.

To apply for Federal DentalBlue coverage, you must be:

• Enrolled in Standard Option and live in one of the following Blue Cross and Blue Shield Plan areas: Alabama or Oklahoma; or.

• Enrolled in Basic Option and live in Alabama or Oklahoma.

You can obtain an enrollment form by calling the local Blue Cross and Blue Shield Plan at the number indicated on the back of your ID card. Complete and sign the enrollment form and return it to the Plan.

Other Options.

Many other Blue Cross and Blue Shield Plans offer dental insurance, not Federal DentalBlue, to Service Benefit Plan members for an additional premium.
