We are pleased to offer you this option to search for a provider near you. Whether at home, in the office, or traveling, this directory will assist you in locating physicians or other professionals, hospitals, pharmacies and other medical facilities throughout the United States.

Preferred Pharmacies are accessed under the Facilities provider type.
Provider arrangements in your Blue Cross and Blue Shield Plan area may change at any time throughout the year. Changes to the Preferred provider listing may include additions or deletions from the Preferred network as a result of provider contract negotiation or termination with the local Blue Cross and Blue Shield Plans. It is your responsibility to verify the Preferred status of your provider prior to receiving services.
If you have any questions about whether a provider is Preferred or a particular service is covered, contact your local Blue Cross and Blue Shield Plan. The Blue Cross and Blue Shield Plan is prohibited from providing Benefits for services rendered by a debarred provider.

Preferred, Participating and member providers file claims for you. If you use a Non-participating physician or a Non-member facility, you may have to file the claim to your local Blue Cross and Blue Shield Plan. Click here to access a claim form.
