Have You Had Your Annual Physical This Year?

If you haven’t had your annual physical in 2011, time is running out to take advantage of our 2011 wellness incentive.  How do you get the wellness incentive?  All you have to do is take the Blue Health Assessment.

Blue Health Assessment
The Blue Health Assessment is a survey that takes about 20 minutes to complete and helps to determine your current health status.  It is a guided evaluation that uses information you provide to help you take steps toward a healthier you.

Once you take the Blue Health Assessment, you will get personalized feedback that includes an action plan and access to our online digital coaching sessions.  You can select coaching sessions that will help you reach your health goals, such as smoking cessation, weight management, and including exercise in your routine.

You can take the Blue Health Assessment online accessed under MyBlue or you can call Blue Health Connection at the number on the back of your ID card to get a printed copy to complete and return.

What Is This Wellness Incentive?
In 2011, once your complete your Blue Health Assessment, you are eligible for your MyBlue Wellness Incentive.  Under Standard Option, we will credit $50 to your calendar year deductible under a self only enrollment or up to $100 under a family enrollment.  If you have Basic Option coverage, you will get a $35 health debit card to use for qualifed medical expenses.  Family enrollments are elgible for two $35 cards.
