Taking good care of yourself benefits you in so many ways. Feeling better leads to greater enjoyment of life; it can also extend your life span. What's more, preventing illness and injury can mean considerable financial benefits. When you're healthier, you can avoid some large medical expenses.
But that's not all. In 2012, the Service Benefit Plan actually rewards you for practicing healthy habits! Take a look:
Earn Your Health Debit Card
The MyBlue Wellness Card is a prepaid health debit card that rewards Standard and Basic Option members for taking charge of their health. Money is added to your card when you complete the following steps:
- Blue Health Assessment
- Up to three online coaching sessions
- Diabetes Management Incentive Program activities
Blue Health Assessment
The Blue Health Assessment, an online health and lifestyle questionnaire, uses your responses to create a personalized health action plan to help you reach specific goals. The questionnaire takes 15 to 20 minutes to complete and helps you identify personal lifestyle risks. The resulting action plan gives you the tools, information and support you need to improve your health.
After you complete the Blue Health Assessment for 2012, you will receive a $35 health debit card to use for qualified medical expenses. Family contracts are eligible to receive two $35 cards when two adult members complete the questionnaire. (For Basic Option members who complete the Blue Health Assessment in 2012, the $35 credit will be applied to your existing health debit card.)
Online Coaching Sessions Incentive
Reaching your health and wellness goals can be hard to do by yourself. That's why you'll get a list of recommended coaching sessions based on your unique needs after you complete your Blue Health Assessment. You can also access the online coaching sessions directly from the Blue Health Connection.
Our comprehensive online digital coaching sessions are free for both Standard and Basic Option members. These coaching sessions provide feedback, advice, encouragement and the support you need to make positive lifestyle changes.
In 2012, you can earn an additional $15 by completing up to three online coaching sessions. For each completed session, you'll receive a $5 credit on your health debit card. (Completion of the Breathe™ module for smoking cessation does not qualify for this credit.)
Diabetes Management Incentive Program Activities
If you have diabetes, it's vital to manage your condition carefully. We want to help make this easier for you.
Our Diabetes Management Incentive Program provides critical information and education; assists with controlling blood sugar; and helps to manage or slow the progression of health complications related to diabetes. With this new program in 2012, you can earn additional credits for your health debit card.
To be eligible for this incentive program, you must be 18 years old or older and complete either the Blue Health Assessment indicating that you have diabetes or the initial questionnaire for the Care™ for Diabetes online coaching session. If you have family coverage, the program is limited to two adult members.
Health debit card credits can really add up when you complete the following activities:
- $10 for getting an A1c test from a covered provider; maximum $20 for two per year
- $5 for reporting A1c levels; maximum $10 for two per year
- $10 for getting diabetic glucose test strips through the Retail or Mail Service Pharmacy Program; maximum of $40 for four fills per year
- $10 for getting a diabetic foot exam from a covered provider; maximum of $10 for one foot exam per year
That's not all! You can also earn credits when you complete one of the following three activities in 2012:
- $20 for enrolling in a diabetes disease management program at your local Blue Cross and Blue Shield Plan OR
- $20 for receiving a diabetic education visit from a covered provider OR
- $5 for completing a web-based diabetes education quiz up to four times per year.
Please note that there is a maximum of $75 in health debit card credits under the Diabetes Management Incentive Program per eligible member in 2012. Once you have earned the maximum of $75, you will not earn additional credits for completing more Diabetes Management Incentive Program activities.
