A comprehensive Continuity of Care Record is included in the PHR which summarizes all the relevant health information contained in your MyBlue PHR. You can print and share this information, providing a more complete medical history to your physician resulting in more efficient medical visits.
MyBlue PHR is simple to use and most of the information is automatically updated. It contains claims submitted by you, your health care providers and your pharmacists from October 2008 to the present time. There is an easy to follow how-to guide online and on-screen help to guide you through the process of adding to or changing your personal MyBlue PHR.
Start by logging on to fepblue.org and click MyBlue in the upper right hand corner. Then select MyBlue Personal Health Record. You will need to register on the site, if you have not done so already. A Personal Identification Number or PIN will be mailed to you within five to ten days. This helps to maintain the security of the site and the confidentiality of your personal health information. When you get your PIN, log back into MyBlue Personal Health Record. You will also be able to sign-on to the Blue Health Connection homepage, which offers access to the other health tools, a multi-media library, chatting online with a nurse, and other health-related features.
