MyBlue Personal Health Record is an online personal health record that allows you to store all of your health information in a central location, with the ability to view, update and print your health information to share with your doctor. When possible, the record is automatically updated from claims records.

Blue Health Assessment^SM

Blue Health Assessment is a quick and easy online health evaluation program. Now when you complete your Blue Health Assessment, you will receive a certificate for a copayment waiver for a routine physical exam or preventive visit with a Preferred provider. Your Blue Health Assessment answers are evaluated to create a unique health action plan. You can also participate in online health programs that can help you reach your health goals.

Jump 4 Health

As part of the Healthy Families program offered through Blue Health Connection, Jump 4 Health offers information to help parents help their children establish healthy lifestyle habits and maintain a healthy weight. For children ages 5 through 17, simply calculate and submit their body mass index (BMI) to determine whether or not they are overweight. If your child meets the criteria to participate, he or she will receive a certificate good for up to 4 free nutritional counseling visits per year. Enroll your child today to learn more.

Online Symptom Advisor

Ever have a health concern that makes you wonder... Should I call a doctor? Is this an emergency? Can I handle this at home? The Online Symptom Advisor asks specific questions about your health concern and uses the same logic your doctor or nurse would use to evaluate the seriousness of the situation. Based on your answers, you get a recommendation for the right care.

Clinical Reference Systems Advisor Series

The Clinical Reference Systems Advisor Series provides information and illustrations for thousands of medical conditions, surgical procedures, and medications. The Series includes an Adult Health Advisor, Behavioral Health Advisor, Cardiology Advisor, Eye Advisor, Medication Advisor, Pediatric Advisor, Senior Health Advisor, Sports Medicine Advisor, and Women's Health Advisor.

Facing the possibility of a chronic condition or long-term illness alone can be overwhelming. That's why long-term health counseling is available. The nurses help you work with your doctor to learn to manage a chronic condition like diabetes or back pain. FEP Local Care Management Programs

The Service Benefit Plan offers local health care management programs to help you manage your health. Depending on your local Plan's service area, you may have access to these programs to manage a chronic condition like diabetes. Local health care management programs support your relationship with your doctor and are designed to improve your health and quality of life. FEP Rare Disease Management Program-AccordantCare

The AccordantCare™ program can help you meet the challenges of living with a chronic condition. The specially trained Accordant nurses will understand your medical condition and are available 24 hours a day, seven days a week, to answer your questions. Accordant Care provides specialized services to FEP Service Benefit Plan members with rare conditions such as; Seizure Disorders, Rheumatoid Arthritis, Multiple Sclerosis, Parkinson's Disease, Systemic Lupus Erythematosus (SLE), Myasthenia Gravis, Sickle Cell Disease, Cystic Fibrosis, Scleroderma, Polymyositis, Chronic Inflammatory Demyelinating Polyradiculoneuropathy (CIDP), Amyotrophic Lateral Sclerosis (ALS), Dermatomyositis, Gaucher Disease.
