Now Service Benefit Plan members in Utah and Oregon can have Disease Management for diabetes, coronary heart disease, asthma, COPD and congestive heart failure at no cost - by phone, 24 hours a day, 7 days a week - through AdviCare.
AdviCare program materials include:
- Introductory Kit that includes a disease-specific workbook to track physician visits, exams and compliance with guidelines for care. Members that have more than one core condition do receive workbooks for each of the conditions managed.
- Telephonic Care Calls for program members Follow-up Care Calls with program members based on member needs
- Nurses available toll-free 24 hours a day, seven days a week
- Mailed reminders about important screening tests and Standards of Care
- An annual health assessment for program members
- Depression screening
- Home blood pressure and weight monitoring for selected program members with heart failure (HF)
- Quarterly, disease-specific newsletter to program members
- Regular communication with providers about the program
- Single-topic educational pieces provided to members on an as needed basis
